Opportunity Community Services is looking for a passionate Housing Case Manager to join our Housing Stabilization Services (HSS) teamHousing Case Managers assist the community through Minnesota Medical Assistance benefit to help people with disabilities, including mental illness and substance use disorder, and seniors find and keep housing.
The Housing Case Manager role is autonomous, with support from team and management, that completes daily documentation expectations, coordinates housing tours, and assists with rental processes, and determines needs and preferences of clients.
This position is responsible for up to 10 clients.An HSS Case Manager would be responsible to plan, and attend housing tours, meet the clients in the community, and document daily housing searches.

Primary responsibilities include but are not limited to:
Develop an individualized person-centered transition plan.
Work with waiver Case Managers to arrange details and provide Bi-Weekly updates.
Conduct one-on-one visits at locations including their homes and offices.
Assist with rental applications, lease agreements, and meeting with landlords.
Help determining eligibility for economic support and research affordable housing.
Assesses and refers individuals, to appropriate community services and resources.
Support and participate with packing items and moving belongings to new location.
Support and assistance in accessing furniture and other household needs.
Secure and inquire moving assistance & vouchers.
The ability to teach individuals the skills needed to live in the community.
Complete assessments of individual and family strengths and needs to develop, implement, and maintain service plans.
Maintain complete, accurate, and up-to-date client records to effectively document case activity and support case decisions.
Provide coordination services by assessing, planning, and authorizing provisions of services in accordance with respective laws, rules, and company policies.
SkillsSkills with internet searches, navigation, and maintaining accurate records.
Proficient in Microsoft Office.
Knowledge of social service programs and an ability to advocate for service.
Knowledge of laws and ethics pertaining to HIPAA privacy and procedures.
Knowledge of socio-economic factors that promote stable family life.
Ability to utilize effective interviewing techniques.
Ability to analyze complex situations and evaluate potential for improvement.
Ability to organize and prioritize a high-volume workload efficiently.
Ability to work independently to follow directions and guidelines.
Ability to seek out resources and information.
Ability to effectively communicate verbally and in writing.
Minimum Qualifications
2-year degree in Social Work, Psychology, Sociology, Human Services, or related field with at least two years of experience.
Must have a good knowledge of community resources.
Can work well with a diverse population: including people with developmental disabilities, low income, and physical disabilities.
Computer proficiency is required.
Ability to be organized and manage time effectively.
Must pass background check administered by DHS.
Must possess a reliable vehicle, active vehicle insurance, and valid Driver's license.
Experience with individuals with disabilities preferred.
