Our mission is to promote and provide affordable housing and tailored services to reduce homelessness and poverty for residents of Snohomish County and Camano Island. We achieve this through a variety of interventions including wrap around services, tailored case management, childrens services, subsidized housing opportunities, adult life skills classes, and access to employment, education, and on-the-job training support.


    Purpose:


    The primary duties of the Family Support Coach are to provide high quality assessment, planning and tailored support services to assist our residents to increase housing stability, income, and family management skills.

    This is a time of transition and opportunity for the Human Services team at Housing Hope.

    We are excited to welcome new team members ideas and contributions as we redefine and shape our services model of support and delivery for our participants.


    Essential Functions:
    To perform this job successfully, an individual must be able to perform each essential function satisfactorily. Assess, orient, and facilitate the entry of individuals and families experiencing homelessness into their new homes.

    Understand the challenges of working with a population affected by homelessness, with a history of domestic violence, substance abuse, gang affiliation and other traumatic circumstances and choices that our residents may face.

    Provide case management and develop individualized family service plans including identification of existing barriers to self-sufficiency, setting housing and income goals, and developing an action plan.

    Evaluate progress and accomplishments of goals regularly, adjusting as needed to improve outcomes. Coordinate and advocate for the involvement of appropriate professionals from other agencies.

    Record case notes of all notable interactions, in a timely manner, with participants including high level conversation topics, goal setting, recommended referrals (internal and external), and overall progress.

    Schedule and conduct community engagement activities on your assigned properties to encourage community building. Maintain accurate and on-going documentation following contract requirements, agency and department policies and procedures. Respond to client emergencies. Participate in and contribute to team building. Attend agency and department meetings. Attend inter-agency and networking meetings as appropriate. Other tasks and responsibilities as assigned and required by program goals and objectives. Assist with other agency requirements as directed. Engages Family Support Coaches/Child and Family Specialist in each pod to gather updates, opportunities, challenges. Report back to Supervisors and Director.

    Facilitates Property Management/Services Meetings for their respective PODS:
    occupancy, move ins/outs, posted notices, delinquencies, evictions, safety, etc.

    Convene weekly POD Family Support Coach/Child and Family Specialist/Behavioral Health Consultation Groups:
    Creates time for collaboration and space for staffing clients as a team.

    Attend Safety Committee Meetings Support staff with Community Engagement in respective PODS New Hire Orientation Carry CaseloadPhysical Demands:

The employee is regularly required to stand; climb or balance; stoop, kneel, crouch, or crawl. The employee must occasionally lift, carry, push, and pull up to 25 lbs. frequently. Items that are over 25 lbs.

    must be lifted, carried, pushed, pulled, or moved with proper equipment for assistance (dolly) and proper safety procedures must also be followed.

    Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.

    Ability to work in a general office environment as well as an outdoor environment. Possible exposure to weatherheat, wet, cold, wind, and noise.


    Requirements:
    Bachelors or Associate degree in social work or related field. 3+yearsworkexperienceorlived experience working with people experiencing homelessness or housing insecurity. Dependable personal transportation is required. Valid Washington Drivers License and proof of automobile insurance required. Must maintain case notes in a timely manner. Computer literacy skills.


    Other Knowledge, Skills & Abilities:
    Be compassionate and open minded to the varying needs of participants. Use trauma informed approaches while accompanying participants on their personal journeys. Be motivational to encourage residents to follow their goals and recognize and celebrate successes at all levels. Critical thinking and problem solving to determine the best plan for each participant.

    Strong written and verbal communication skills to communicate with residents of all ages and backgrounds, external partners, and other departments in the agency.

    Flexibility to manage a case load of varying needs, backgrounds, experiences, and situations. Ability to organize and prioritize multiple tasks. Ability to work with individuals of varying skills and backgrounds. Ability to work as part of a team. Ability to work some evenings.
