    Position Summary


    The primary purpose of your position is to plan, organize, develop and direct the overall operation of the Wellness Department and to ensure the delivery of the highest quality assisted living services focused on maximizing residents' individual capabilities in accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility, and as may be directed by the Executive Director.

    This is a full-time, exempt position, which often requires work beyond normal business hours, and occasional weekends and holidays.

    This position requires the incumbent to be on-call 24 hours a day, 7 days a week and is subject to call-back during emergency conditions (e.g., severe weather, evacuation, post-disaster, etc.).

    Essential Functions and Responsibilities


    • Ensure the CAPLICO Core Values are adhered to at all times.
    • Plans, develops, organizes, implements and evaluates resident care goals.
    • Ensure that all residents receive quality care and maximize resident's individual capabilities.
    • Ensure that resident's rights are well established and maintained at all times.
    • Ensure an environment of teamwork and cooperation between resident care department employees/other departments.
    • Maintain the confidentiality of all pertinent resident care information to ensure resident rights are protected.
    • Assist in the development and implementation of departmental policies and procedures.
    • Ensure effective, efficient and adequate staffing of the resident care department by creating a work schedule that meets the needs of residents 24/7, while being mindful of staffing budget.
    • Ensure that nursing interventions meet the personal, physical and cognitive needs of each resident as well as maximize his/her self-care capacities, identify, independence, choice and opportunity for social interaction.
    • Ensure that all applicable Ensign/Bridgestone Human Resources policies and procedures and State and Federal employment laws and guidelines are followed by all resident care department employees.
    • Ensure that all caregivers are appropriately oriented, on-boarded and trained, according to company policy and procedure.
    • Ensure all caregivers complete required training and in-services according to established timeframes.
    • Ensure employee and resident health and safety programs are implemented to provide a safe environment.
    • Actively maintain cohesive public and professional relationships with residents, families, physicians and the community.
    • Maintain ongoing education as it relates to continuing licensure and maintenance of professional skills, if appropriate.
    • Work with Executive Director on Wellness department budgeting and spend downs; ensure adherence to established department budget while also ensuring that quality of resident care is not affected.
    • Work with Executive Director to promptly investigate and properly document any allegations of abuse or neglect. Assure prompt notification to resident's physician and responsible parties.
    • Monitor and review resident incidents to identify possible legal issues and to identify patterns that may require preventive intervention.
    • Ensure resident care staff is educated and utilizes infection control practices that protect staff, residents and families.
    • Periodically visit all Caregiver shifts to monitor performance of staff and to ensure quality care is being provided to residents.
    • Assume full responsibility for the operation and management of the facility in the temporary absence of the Executive Director or as directed by the Executive Director.
    • Other duties as deemed necessary and appropriate, or as may be directed by the Executive Director.
    • Communicates resident care and service needs regularly to family.
    • Completes resident assessments for resident level of care and appropriate placement.
    • Develops, implements and updates Service Plans.
    • Assists with new resident orientation and transition to community.
    • Meets with residents regularly to encourage communication and gain understanding of their needs.
    • Conduct daily rounds to review performance of resident care staff.
    • Create and monitor service records for all residents.
    • Gather medical and contact information for all new residents.
    • Review resident chart documentation for completion and accuracy.
    • Participate in resident/family conferences.

    Assist/oversee schedule ancillary services:
    podiatry, dentist, therapies, pharmacy consultation, etc.


    • Participate in Resident Retention Program.
    • Continuously prepare and ensure readiness for survey; conduct follow-up to ensure corrective actions appropriately completed.
    • Ensures documentation of resident clinical status and needs communication to physicians and other healthcare professionals in resident record.
    • Provide hands-on care as staffing needs require (and licensure allows).
    • Conduct visits with residents during hospitalizations and short-term rehabilitation admissions.
    • Assist with discharge planning and move-outs.
    • Ensure compliance with established procedures when a resident expires.
    • Oversee medication program including accuracy of medication assistance records (MARs), providing hands-on medication assistance to residents and monitoring medication pass.
    • Provide communication and follow-up with provider pharmacy to ensure compliance with resident medication orders.
    • Conduct QA audits of medication records and med storage areas with communication of findings to Executive Director.
    Supervisory Responsibilities

    Exercises administrative and functional supervision over all Wellness Department employees. Must be responsive to management, physicians, staff and resident needs.

    • Is involved with residents, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstances.
    • Is subject to hostile and emotionally upset residents, family members, personnel, and visitors.
    • Is involved in community/civic health matters/projects as appropriate.
    • Is subject to injury from falls, burns from equipment, odors, etc., throughout the work day, as well as to reactions from dust, disinfectants, tobacco smoke, and other air contaminants.
    • Is subject to exposure to infectious waste, diseases, conditions, etc., including TB and the AIDS and Hepatitis viruses.
    • May be subject to the handling of and exposure to hazardous chemicals.

    • Must be able to function independently, have personal integrity, flexibility and the ability to work effectively with residents, employees, visitors and support agencies.
    • Must be in a good general health and demonstrate emotional stability.
    • Must be able to relate and work with the disabled, ill, elderly, emotionally upset and at times, hostile people within the facility.
    • Must be able to lift, push, pull and move a minimum of 50 pounds.
    • Must be able to assist with the evacuation of residents.

    • Follows written and oral instructions.
    • Knowledgeable in the use of computers, e-mail, spreadsheets and document word processing.
    • Maintain patience, tact, cheerful disposition and enthusiasm, as well as ability to handle residents, staff, and visitors, based on whatever maturity level they are currently functioning.
    • Ability to travel for training and work related activities.
    Minimum Education and Experience Requirements


    • Must possess, as a minimum, a graduation certificate from an approved LPN/LVN program; Nursing Degree from an accredited college or university is preferred.
    • Must have current CPR certification
    • If required by your state of residence, must possess a current, unencumbered, active license to practice as a Nurse.
    • If required by your state of residence, must possess a current First Aid certification.
    • Prefer, as a minimum, 4 year(s) of experience as a supervisor in a healthcare setting; hospital (geriatrics), skilled nursing or assisted living facility.
