- Desire to be part of the development of a world-class Endocrinology program and team.
- Excellent communication (written and verbal)Opportunity Details:
- Palliative Care Team currently consists of three (3) Physicians and four (4) Nurse Practitioners, Social Worker and Chaplain
- Schedule is Monday through Friday; 8am-5pm
- This is an outpatient opportunity as we are looking to expand our outpatient role in the community.
- Will work collaboratively with physician, nurse practitioners, other consultants and primary team, as well as an interdisciplinary team including Social Worker, Case Management and chaplain
- Will participate in the initial consults, develop and implement care
- Community education and outreach
- Call is 1:5
- EMR Platform – Integrated Epic/EHR with HIMSS Stage 7 validation
- Exceptional employed Hospitalist & Critical Care team, consisting of over 125 providers, offers 24/7 support
- Opportunity to work with our Hospice and Palliative Care fellows **The ideal candidate will possess a mature, warm and patient personality with a passion for palliative medicine and care. Flexibility and compassionate bedside manner are essential. Must be a self-starter who takes initiative to work collaboratively with Interdisciplinary Team.
