The Clinical Pharmacist Specialist will collaborate with physicians and staff to provide coordinated, comprehensive, high quality care to patients.

Bachelor's Degree In PHARMACY from an accredited School of Pharmacy. Or
PharmD In PHARMACY from an accredited School of Pharmacy.
Coursework/Training Postgraduate Year One (PGY-1) AND Year Two (PGY-2) Pharmacy Residency or equivalent experience Or equivalent educ/experience
Upon hire:
Oregon Pharmacist License
Within 2 years of hire Certification from Board of Pharmacy Specialties
TWO years of Senior Ambulatory Clinical Pharmacist experience within relevant practice setting - OR - FIVE years of pharmacy practice experience with at least 2 years of experience in relevant setting in lieu of PGY-1 and PGY-2 residency requirement.

Board of Pharmacy Preceptor license when eligible and where applicable
