Job summary
The Hospice Social Worker MSW is responsible for providing quality medical social work services to patients and families in accordance with company standards and federal state and local regulations.The job requires ongoing assessment of patients assuring an accurate evaluation of patient/family needs developing a comprehensive care plan that addresses the psychosocial needs of the patient and family making home health visits as needed working with patients and families regarding coping with illness accessing resources or other issues affecting the patient's health status linking patient/family with needed services assisting patients in arranging for services and funeral planning supervising cosigning all care plans created by Licensed Social Workers LSWs planning for participating in Interdisciplinary Team meetings collaborating Interdisciplinary Team members regarding crisis situations and on-going care of patient/family evaluating bereavement risk at start of care continued evaluation bereavement needs following current social work professional standards practice agency approved protocols monitors psychosocial-care plans makes changes to care plans as needed maintains current accurate patient records thorough use laptop desktop computers electronic medical record paper/chart documentation accordance company policies federal state local guidelines maintains knowledge requirements regulatory agencies accrediting bodies third party payers participates quality improvement projects as needed develops maintains close working relationship community resources referral sources markets social work/care management services achieve volume goals provides direction orientation inservice continuing education home healthcare staff requested keeping staff current changes healthcare procedures policies needed commitment quality customer service internal external customers performs other duties assigned qualifications education Master degree social work accredited Council Social Work Education required unencumbered license practicing Licensed Social Worker LSW state Ohio required experience one year experience social work healthcare setting proficiency Windows Microsoft Office Word Excel PowerPoint internet required other requirements able read write speak understand English language valid driver license acceptable driving record insurable under professional liability crime coverage policies specified insurance carrier underwriting standards working conditions special require sitting standing walking lifting transferring pushing pulling patients equipment supplies driving weekends evenings holidays Required scheduled on call availability 247 emergencies subject patients various disease processes occasional may exposed infectious waste disease conditions including exposure AIDS Hepatitis B viruses Risk Category Exposure Bloodborne Diseases II
