    The Licensed Practical Nurse (LPN) in Home Health plays a crucial role in delivering high-quality patient care and providing technical skilled services in patients' homes.

    • Delivering services within the scope of nursing practice as defined by state regulations, following the care plan, and collaborating with other healthcare team members. Any changes to the care plan must be authorized.
    • Training the patient's family on caregiving and maintaining a healthy environment.
    • Monitoring, documenting, and reporting the patient's progress and any changes in their condition to the supervising nurse or physician on a daily basis.

    Requirements for licensure:

    • Active LPN license in the state of practice
    • Valid Driver's License, auto insurance, and access to a reliable vehicle or public transportation
    • CPR Certification
