    Ability to providePerson Centered, Family-Centered, and Trauma-Informed Care; Understanding and commitment to supporting recovery and resiliency; Ability to assess and interpret information related to population; Knowledge of the symptoms of the disability/disorder and ability to recognize exacerbation of symptoms; General understanding of treatment needs; Knowledge of available treatment resources and appropriate use; Population-specific communication skills; Understanding of Care Coordination; General knowledge of housing subsidies and management of the subsidies

    Responsibilities:


    Provides community-based services to consumers and consumer's family in the community;Facilitates service linkage in the mental health and non-mental health, health system; coordinating and integrating services from multiple providers;Assists and supports individuals in developing skills to gain access to needed medical, behavioral health, housing, employment, social, educational and other services essential to meeting basic needs;Provides training and/or direct service support to individuals in the use of basic community resources such as transportation and financial resources;Provides necessary support to ensure that individuals needs and goals are addressed, including aggressive advocacy, escort to appointment, daily living skill remediation and money management;Provides accurate and timely documentation of services in the electronic health record;Provides 24 hours/7 days per week on call crises intervention, including in person response on a rotating basis, including evenings and weekends;Works collaboratively with the Clinical Team;Driving and travel throughout the County required Benefits:Competitive base salaryMedical and dental insuranceVision planRetirement planFlexible spending plansEXCELLENT time benefits for qualified positionsOpportunity for personal and career growth, including supervision towards professional licensure for eligible candidatesTeam-oriented environment - we practice the FISH PhilosophyQualifications:Bachelor's degree in a related field1 year experience working in a behavioral health setting preferred;Valid Driver's LicenseAll positions require a valid driver's license in good standing and pre-employment drug screening.
