- Loading members from a computer system to a spreadsheet.
- Making outreach to members to attempt close care gaps of care in specific areas by assisting in making PCP appointments and finding PCP if needed.
- Also assisting with finding resources for SDOH. Documenting in various systems the work that has been done with a member.
- Meet weekly call goal of 250 calls, 50-60 members in panel daily and meet monthly audit scores.
