    The WOC Coordinator is responsible to the Clinical Director for supervision of the Agency's service programs and patient care activities on the weekend.

    Maintains and promotes quality of patient care, in accordance with the Agency objectives and policies. This is an Exempt position.

    Responsibilities


    • 1. The following description of job responsibilities and standards are intended to reflect the major responsibilities and duties of the job but is not intended to describe minor duties or other responsibilities that may be assigned from time to time.
    • 2. Weekend coordination and management of all services rendered to patients and families by direct care service and contracted staff; oversees maintenance of accurate record system. Evaluates effectiveness of programs and recommends appropriate modifications.
    • 3. Reviews all available patient information including home visits for assessment of needs and assignment of personnel initially and on an ongoing basis.
    • 4. Assures weekend assignments are distributed effectively in order to maintain staff productivity and efficiency.
    • 5. Counsels staff appropriately regarding individual performance.
    • 6. Models appropriate professional appearance and behavior.
    • 7. Maintains client confidentiality and abides by "Patient's Rights and Responsibilities".
    • 8. Performs other duties and responsibilities as assigned by the Clinical Director and/or VNA President.
    • 9. Projects a positive attitude and enthusiasm about agency mission, philosophy, and policies.
    Qualifications


    EDUCATION


    M.S.N., current Connecticut RN license, and a minimum of one year full-time clinical experience in a home health agency which included care of the sick or B.S.N., current Connecticut R.N.

    license, and three years of full-time clinical experience in nursing, at least one of which was in a home health agency which included care of the sick.

    PHYSICAL DEMAND

    Medium to heavy work:
    constant handling of under 10 pounds to occasional handling of 70 pounds. Frequent driving required. Standing, walking and handling are performed on a constant basis. Sitting, carrying, bending, lifting/lowering, pushing/pulling with whole body and with arms are performed on a frequent basis. Squatting/kneeling, twisting, and reaching are performed on an occasional basis.

    The highest physical demands of this position are incurred while transferring client requiring maximal assist of one or more persons, for example, side lying to sitting, sitting to standing, and lifting from chair, commode, toilet, or bed.

    Loads lifted or forces exerted up to 70 pounds for client weighing up to 220 pounds in height range of 15-36 inches.
