- Conduct and document an initial assessment of the patient and family to identify home care needs;
- Initiate and adjust the care plan as the patient's condition and needs evolve;
- Administer medications and treatments as per physician's instructions;
- Provide guidance, supervision, and assessment of home health aide services every two weeks;
- Determine discharge planning needs during care plan development;
- Adhere to confidentiality standards established by the agency and Federal HIPAA guidelines;
- Demonstrate proficiency in OASIS documentation;
- Complete documentation via EPIC following agency timeliness policies.
- Active and valid Massachusetts RN license;
- Current American Health Association BLS certification;
- Associate's Degree or BSN (preferred) from an accredited nursing school;
- Minimum 1 year of direct nursing care experience, ideally in public health or home care nursing; OASIS and/or EPIC background is advantageous.
