    Description

    The Nurse Practitioner works in collaboration with the Medical Director and multi-disciplinary team to deliver comprehensive and compassionate hospice and palliative care and services to patients diagnosed with terminal or life-limiting illness in all settings.

    Provides face to face visits, rounding in inpatient centers, and symptom management visits.


    Key Responsibilities:

    • Conduct comprehensive assessments of patients, diagnose and manage symptoms.
    • Develop individualized care plans in collaboration with the interdisciplinary team, patients, and their families. Establish goals of care based on patient preferences, values, and treatment options.
    • Prescribe and manage medications to alleviate symptoms and improve the patient's comfort.
    • Communicate effectively with patients, families, and other healthcare team members.
    • Provide education on the nature of the illness, treatment options, and end-of-life care decisions.
    • Maintain accurate and thorough medical records, documenting assessments, care plans, and interventions. Ensure compliance with regulatory standards and guidelines.
    • Shift time: Monday-Friday, 8 am to 4:30 pm, with evening and weekend on call responsibility as scheduled

    Skills:

    • Strong clinical assessment and diagnostic skills.
    • Excellent communication and interpersonal skills.
    • Compassion and empathy in dealing with patients and their families.
    • Ability to work collaboratively in a multidisciplinary team.
    • Knowledge of ethical issues related to end-of-life and palliative care.
    • Graduate of an accredited School of Nursing, completion of post-basic program for advanced nursing practice and board certification
    • Current RN/APRN licensure and DEA licensure in Ohio and Kentucky
    • Knowledge of current trends and techniques in nursing practice
    • At least one year of experience in a patient care position in a hospital, nursing home, or related health care facility preferred PI1e31460bee
