    Job Purpose


    Working within RN scope of practice, collaborates with providers and supportive staff to coordinate all aspects of patient care, ensuring adherence to department policies, and nursing procedures, standards, and practices.

    Essential Job Functions

    Responsible for the general coordination of the primary physician's patients.
    Provide continuous and comprehensive education and information to patients and families.

    After 1 year, may be asked to assist with weekend/holiday calls and occasional weekend/holiday treatments in rotation with other Nurse Navigators.

    Independently perform telephone assessment using the nursing process and apply judgment and decision- making skills to coordinate a plan of care for the patient.

    Document all telephone assessment calls according to protocol.
    Demonstrate knowledge, skill, and coordination to provide nursing care and guidance to the patient from diagnosis to survivorship.
    Act as a liaison between infusion nursing and providers, answering questions at the highest level within scope of practice.
    Make referrals to appropriate supportive care services as needed.
    Ensure that patients requesting prescriptions are handled according to protocol including documentation and delivery.

    Follow MD's hospital patient list to anticipate schedule changes for office, test results, discharge planning and communication with other participating physicians.

    Assist in the coordination of end of life care for patients and families and provide emotional support.
    Ability to be at work regularly and on time.
    Other duties as assigned.

    Qualifications

    Must be able to maintain emotional stability in order to cope with human suffering, emergencies, and other stresses.
    Must have knowledge of oncology treatment procedures, facilities and equipment.
    Must have knowledge of supportive care principles and techniques for oncology patients.
    Must have knowledge of related accreditation and certification requirements.

    Must have ability to educate patients and/or families as to the nature of diseases and provide instruction on proper care and treatment.

    Must have the ability to perform nursing assessments and patient triage.

    Must have knowledge of the nature and treatments of oncologic diseases and of reactions and side effects of related therapy methods.

    Must have patient assessment and referral skills.
    Must have knowledge of bereavement counseling techniques.
    Must have ability to maintain quality, safety, and/or infection control standards.

    Education/Experience

    Must have Alabama RN license.
    BLS and chemotherapy certification/training preferred.
    Must have at least (3) years' nursing experience with at least (1) year acute care or oncology experience.

    Working conditions


    Environment involves exposure to potentially dangerous materials and situations that require following extensive safety precautions and may include the use of protective equipment.

    Physical requirements

    Light physical effort.
    Requires handling of average-weight up to 10 pounds and frequent standing or walking.
