Summary:

The Primary Care Physician provides primary care and continuous medical coverage services to participants. Demonstrates the knowledge and skills necessary to assess, plan, care for, and provide service in accordance with PACE Program standards.

Essential Duties and Responsibilities:

Manages all care of participants during acute illness and in the skilled nursing facility.  Provide regular visits as dictated by the plan of care and participant's needs. Makes telephone contact with hospital and/or with skilled nursing facility staff as required or needed; Admits skilled nursing home participants to hospital when necessary

Performs in-person comprehensive history and physical on prospective or new participants

Coordinates with the interdisciplinary team to develop a comprehensive plan of care for each participant

Conducts an in-person reassessment semi-annually and as needed

Oversees, authorizes, and manages participant's use of medical specialist and inpatient care based on PACE care pathways and the treatment goals of the patient

Admits participant to the hospital. Provides primary care responsibilities for medical management or coordination of care with Hospitalists and provide updates to Interdisciplinary team. Refers participants to medical specialist as indicated, communicates, and follows up with specialist regarding outcome of visit in accordance with PACE care pathways and the treatment goals of the patient

Works collaboratively with the Nurse Practitioner(s) and the entire interdisciplinary team

Participates in rotating after hours and weekend call

At the direction of the Medical Director, reviews medical outcomes/statistical and utilization data, including review of necessity of hospitalizations and use of specialists; When problems are identified, works with the Medical Director and the QAPI team to establish best practices and with clinical staff and interdisciplinary team to avoid unnecessary hospitalizations and improve the health of participants

Maintains up-to-date and appropriate documentation in the participant's electronic medical record

Participates in the interdisciplinary team meetings, communicating participant changes, collaborating on care planning decisions and coordination of 24-hour care delivery

Qualifications, Knowledge, Skills, and Abilities:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.  The requirements listed below are representative of the knowledge, skill and/or ability required. 

-Medical degree: MD or DO

-Current State Physician license

-Board Certified – Internal Medicine or Family Practice

-State CDS Certification and DEA Registration

-(2) years working with the elderly population

-Medical clearance for communicable diseases and up-to-date immunizations, before having direct participant contact

-Current driver's license, auto insurance, and dependable transportation

-Thorough knowledge of current concepts, theories and practices related to home and community-based care for the elderly and disabled adults

-Working knowledge of health care delivery and financing systems including Medicaid, Medicare, waiver programs, prospective and capitated systems, public health programs and HMO/Managed Care

-Thorough knowledge of physical, mental, and social needs of frail older adults

-Working knowledge of PACE regulations

-Ability to meet position specific competencies prior to assuming participant care

-Ability to work effectively within an interdisciplinary team model, interfacing and collaborating with a wide range of clinical and social services disciplines who work together to manage the PACE participants' care.

-Effective oral and written communication skills

-Effective interpersonal skills with ability to build relationships

-Effective organizational skills with attention to detail and time management skills

-Strong analytical and problem-solving skills

-Proficient with Microsoft Office Suite or related software

-Proficient with Electronic Medical Records

Physical Activities and Demands; Working Environment:

-Prolong periods of standing and walking; occasionally sitting

-Ability to squat, crouch, bend, kneel, and crawl occasionally

-Ability to push, pull, reach above shoulders occasionally

-Ability to frequently lift/carry up to 25 pounds, occasionally lift/carry up to 50 pounds using appropriate body mechanics with reasonable accommodation if needed

-Frequently use hands for simple and firm grasping and fine manipulation

-Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus

-Exposure to bodily fluids, bloodborne pathogens, and/or other infectious illnesses common in a healthcare environment

-Exposure to hazardous chemicals such as disinfectants, cleansers, soaps, etc.

-Exposure to fluctuating indoor temperatures

-Noise level in the work environment is usually moderate to loud

-Subject to unpleasant odors

-Potential exposure to verbal aggression by participant

-30% of the time in participant homes, nursing facility or hospital, balance at New Horizons Living Center
