    Job Description:

    Foundation Medical Partners is seeking a compassionate and dedicated Geriatrician to join our team and provide part-time comprehensive medical care to our residents in a vibrant and supportive nursing home environment.

    As a Geriatrician with us, you will play a vital role in ensuring the well-being and quality of life for our older adult population.

    Conduct comprehensive initial and ongoing evaluations of residents' medical and psychosocial needs.
    Diagnose and treat a wide range of acute and chronic medical conditions common in older adults.
    Develop and implement individualized care plans in collaboration with nursing staff, therapists, and other healthcare professionals.
    Manage residents' medications, ensuring safe and effective use.
    Provide family members and caregivers with education and support regarding their loved ones' care.
    Participate in interdisciplinary care conferences and contribute to quality improvement initiatives.
    Stay up-to-date on the latest advances in geriatric medicine and evidence-based practices.
    Document medical records accurately and efficiently.


    Qualifications:
    Board-certified or board-eligible in Geriatric Medicine or Internal MedicineValid medical license in the state of NH (or working towards)
