Rocky Mountain Human Services (RMHS) is a nonprofit organization dedicated to providing person-centered case management and direct services to enhance the well-being, self-sufficiency, and overall quality of life for children, adults, and veterans across Colorado.

Position Purpose

Case managers serve as the main contact for members receiving Long Term Services and Supports through Medicaid or State Funded programs.

The case manager provides specialized, ongoing case management by monitoring services, having communication with members and providers, and coordinating resources and requests as needed.

Case managers serve as the experts in eligibility assessments, service plans, and prior authorization requests (PARs).

This is a full-time hybrid Case Manager role will identify and address the needs of individuals by coordinating care plans, connecting clients to relevant services, and maintaining accurate records.

Responsibilities include conducting assessments, monitoring service plans, advocating for clients, and collaborating with other service providers. While some in-office work is required, this position offers flexibility with primary work-from-home opportunities.

Qualifications
Strong case management, client advocacy, and problem-solving skills
Excellent communication, collaboration, and interpersonal skills
Proficiency in documentation, data management, and maintaining accurate case records
Organizational and time management skills to prioritize tasks effectively
Communicates professionally and empathetically both verbally and in writing.
Knowledge, understanding and competency about members who have disabilities and members who are elderly.
Maintains confidentiality per HIPAA guidelines.
Prior experience working with a vulnerable population.
Ability to manage your own schedule and work hours to effectively complete tasks assigned.
Ability to manage multiple priorities and work in a fast-paced environment.
Ability to speak calmly and help de-escalate members who may be upset by showing care and compassion.
Ability to solve problems and concerns as they arise.
Ability to attend in-person meetings with members at their residence.
Knowledge of basic computer skills and ability to navigate client management systems.
Ability to represent RMHS's mission, vision, and values.
Knowledge or experience in mental health services, social work, or human services is a plus
Proficiency with technology and familiarity with case management software

Bachelor's degree in Social Work, Human Services, Psychology, or a related field preferred, or
Five (5) years of relevant experience in the field of LTSS, which includes Developmental Disabilities; or
Some combination of education and relevant experience appropriate to the requirements of the position.

Examples of preferred experience:
preschool, assistant living, CNA, service coordinator, direct service provider.

Preferred Qualifications
Previous case management experience
Degree in social work, psychology, sociology or other human services
Spanish Speaking
Ability to work in a very fast paced, often stressful environment.
