    We are seeking a dedicated and compassionate LPN to join our team at Rose Senior Living Avon. As an integral part of our healthcare team, you will provide direct nursing care to residents, delegate and supervise the care provided by Quality of Life Specialists/Resident Assistants, and promote independent lifestyles.

    Daily Responsibilities

    • Assist residents with Activities of Daily Living (ADL), family/social activities, and actively promote independent life-style of residents.
    • Identify resident medical and behavioral changes, develop and monitor care plans, and reassess each resident as needed.
    • Provide daily reports to the Director of Health Services concerning community conditions, personnel performance, resident conditions, and needs for nursing supplies.
    • Deliver nursing care according to physician's orders and established standards.
    • Coordinate nursing care with other disciplines to ensure preventative, therapeutic, and rehabilitative services to residents.

    Requirements

    • Current nursing license valid in the state where worked.
    • Availability for emergencies regarding the community.
