Responsibilities include performing cross-functional duties such as pharmacist clinical specialist or other clinical coordinator as required Demonstrates ability to perform clinical/technical/service/administrative tasks Conducts participates in departmental activities to meet goals objectives and policies of the department hospital/medical center by standards set forth by Federal State Regulatory agencies and The Joint Commission Develops implements policies procedures to meet professional standards improve medications safety including established by American Society of Health-System Pharmacists ASHP Implements decisions provides support for Pharmacy & Therapeutics committee preparing drug evaluations assisting formulary maintenance Provides drug information investigational protocol education to pharmacy staff students health care professionals Plans organizes implements specific site policies regarding Investigational Drug Services Maintains patient/employee confidentiality managing information Coordinates provision pharmaceutical care pharmacy department members Ensures work environment well maintained Demonstrates initiative leadership Remains current various pharmaceutical databases national guidelines evidence-based medicine medication therapy management disease states Provides education patients family members caretakers about disease states medications Desired results/goals chronic disease states Name strength description purpose each medication How administer timely safely appropriately Agents work attain adequate therapy Interacting foods disease drugs appropriate Importance therapeutic drug monitoring efficacy toxicity Recognition disease exacerbation procedures follow instances Recognition side effects therapy importance notifying physician occur Importance adherence pharmacotherapy appointments Aids increase medication adherence
