    INTRODUCTION:

    The Community Health Advocate's (CHA) provides a variety of services to tribal members relating to healthcare. CHAs advocate for individuals and help identify services available for their communities and tribal members, and promote wellness for individuals and the community as a whole.

    DUTIES:

    • Reduce barriers to Tribal members receiving health services including:
    • Serving as liaison and/or referral source between clientele and relevant tribal programs, healthcare providers, and county, state, and federal agencies.
    • Providing advocacy for client as needed.
    • Providing or arranging transportation in accordance with tribal transportation policy.
    • Provide health information and activities to the Tribal membership in local service area through monthly newsletter submissions, quarterly health promotion activities, and periodic health screenings.
    • Serves as local distributor and educator of health and safety equipment including bicycle helmets, car seats, and Elders Safety Project items per department policy.
    • Consults with supervisor in a timely fashion regarding situations outside the scope of CHA knowledge and experience.
    • Works with Purchase Referred Care to provide pre and post operative medical related services.
    • Contacts assigned elders regularly to offer services, keeps assigned elder's list updated and contacts are tracked in real time.
    • Works closely with supervisor to provide local service area clientele access to Community Health Department sponsored events.
    • Provides nutritional education support to USDA Program (Siletz and Salem CHAs only).
    • Maintains accurate and current client records and provides quarterly and annual program reports as required.
    • Tactfully encourages wellness practices as well as client follow-up on existing health problems.
    • Support the diabetes program.
    • Dispense and maintain over-the-counter medication inventory.
    • Support the Healthy Traditions Program as needed: Nutrition cooking classes, Food preservation: freezing, drying, canning.
    • Other duties as assigned.
    CHA Providers will document and track services provided for billing purposes. They will remain current on trainings that allow their services to be billable and will submit billing regularly.

    Requirements

    REQUIREMENTS:

    CHA I: High School diploma or GED and one year of health-related coursework or experience working with the public in a health or social service related field.

    CHA II: Certified Basic Nursing Assistant or Certified THW/Community Health Worker, and one year experience working with the public in a health or social service field.

    CHA III - Provider: One year serving as a CHA, certified THW/Community Health Worker, active NPI and be able to bill for services.

    Knowledge of:
    • Healthcare systems and payment mechanisms utilized by Siletz tribal members, including Purchased Referred Care, Oregon Health Plan, and IHS medical facilities.
    • Local, private, and county healthcare service systems and individual medical providers.
    • Basic preventive health practices in the areas of nutrition, tobacco, alcohol, and drug use, physical activity, communicable disease, maternal-child health, and routine health screenings.
    • A basic understanding of disease and illness, and skill sufficient to teach clients, and to impress upon them and their family members the necessity to continue the procedures of proper health care.
    • The unique health concerns of Native Americans, preferred.
    Ability to:
    • Model preventive health practices during work hours.
    • To travel locally and long distance in accordance with tribal transportation policy.
    • Build and maintain client rapport and trust through a nonjudgmental attitude, advocacy, timely follow-up to client requests, and unconditional positive regard.
    • Maintain personal and professional boundaries. Therapeutic boundaries in interactions with clientele are necessary to maintain professionalism and to ensure proper utilization of community health staff.
    • Exercise good judgment in situations significantly impacting client and/or own safety and well-being.
    Other:
    • Obtain First Aid/CPR within the first 30-days of hire.
    • Obtain Food Handlers card within 15-days of hire.
    • Complete Motivational Interviewing basic course within 6-months of hire.
    • Must possess a valid driver's license, insurance and be eligible to drive a GSA Vehicle.
    Salary Description

    $20.45/hr. - $26.08/hr.
