- Prior experience in skilled nursing, internal medicine, family medicine, geriatrics, cardiology, or nephrology is strongly preferred.
- Proven ability to collaborate with facility staff, including nursing directors, executive leadership, and other advanced practice providers.
- Commitment to ongoing clinical education and a proactive approach to patient care.
- Compassionate, patient-centered approach to managing chronic and acute conditions in a vulnerable population.
- Deliver high-quality Cardiology and Nephrology care to Skilled Nursing residents
- Collaborate with facility and primary care leadership
- Provide education and support to facility staff on relevant clinical topics
- Meet and maintain monthly encounter goals
- Serve as a clinical resource in cardiology and nephrology for the care team

We are currently seeking a motivated and compassionate Nurse Practitioner (NP) or Physician Assistant (PA-C) to join our team in the Tacoma, WA area.
