    We are seeking passionate and skilled Nurse Practitioners eager to make a significant impact in their communities through their clinical expertise.

    Your Role

    As a Nurse Practitioner at Matrix, you will have the rewarding opportunity to conduct detailed health assessments during visits of up to one hour. You will closely observe and evaluate each member's health status, medical history, medication adherence, and social environment-insights often overlooked during standard office visits. Your key responsibilities will include:

    • Performing comprehensive adult and geriatric assessments, including medical history reviews, diagnosis formulation, treatment planning, health education, and necessary referrals.
    • Administering and ordering point-of-care and lab-based tests as required.
    • Collaborating with a diverse team of physicians, case managers, social workers, caregivers, and medical personnel.
    • Engaging with Primary Care Physicians (PCP) for patient education and follow-up support.
    • Providing care in members' homes or Skilled Nursing Facilities.

    Key Qualifications

    • A Master's Degree or equivalent experience along with NP licensure.
    • Current NP licensure in your practice state, with prescriptive authority or eligibility to obtain it.
    • Board certification from AANP, AACN, or ANCC in an accepted specialization.
    • Current BLS, ACLS, or CPR certification.
    • A minimum of one year of NP experience is preferred, though new graduates are highly encouraged to apply.
    • Willingness to travel up to 15% of the time, including potential overnight trips.
    • A valid driver's license and automobile insurance meeting state requirements.
    • Strong computer skills and familiarity with medical record software.
    • Excellent communication skills, both written and verbal.
