Job Summary
Diligent Home Health Care is hiring a Home Health Scheduler to coordinate caregiver visits, ensure timely admissions, and support seamless in‑home care across the greater Jacksonville service area.
You will match caregivers to client needs, manage daily schedules in the EHR, communicate with clinicians and families, and quickly resolve staffing gaps.
