    Job Summary
    Coordinates record review and assists requesters with access to and release of information.

    Responds to requests for medical information by performing duties in accordance with established hospital and departmental policy and state and federal laws.


    Duties and Responsibilities
    Responsible for coordinating the release of medical information to all requesters, including patients, physicians and other healthcare providers.

    Responsible for coordinating the release of medical information to insurance companies and lawyers.

    Responsible for processing of subpoenas and court orders.

    Verifies authorizations in accordance with hospital policy and procedures and state and federal laws.

    Responsible for maintaining security and confidentiality regulations for record access and review. Consults director or supervisor of any unusual request.

    All requests for records are stamped with date received and logged in the correspondence computer.

    Receives all calls to the department and routes them appropriately.

    Receives all visitors to the department and helps/routes them appropriately.

    Maintains supply closet and orders supplies as necessary.

    Performs performance improvement functions through data collection and documentation review.

    Supports and maintains a culture of safety and quality.

    Maintains a good working relationship within the department and other departments.

    Assists in retrieving medical records for physician completion, when necessary.

    Willing to accept additional assignments.

    Adheres to dress code, appearance is neat and clean.

    Completes annual education requirements.

    Maintains regulatory requirements, including all federal, state, local regulations and accrediting organization standards.

    Maintains patient confidentiality at all times.

    Reports to work on time and as scheduled, completes work within designated time.

    Wears identification while on duty, uses computerized punch time system correctly.

    Gathers paper charts from various departments every business day.

    Analyzes charts for completion and assigns deficiencies as needed.

    Responsible for scanning paper records into corresponding patient charts.

    Represents the organization in a positive and professional manner.

    Actively participates in performance improvement and continuous quality improvement (CQI) activities.

    Complies with all organizational policies regarding ethical business practices.

    Communicates the mission, ethics and goals of the hospital.

    Performs other duties as assigned.

    Requirements


    Minimum Requirements

    High School graduate or equivalent.

    One (1) or more years of previous similar hospital experience.


    Preferred Requirements

    Associate's degree in a related field of study.

    Ability to speak additional languages.


    Skills/Competencies
    Able to communicate effectively in English, both verbally and in writing.

    Basic computer knowledge.

    Excellent interpersonal skills.

    Self-disciplined and self-motivated to complete required job duties in a timely and accurate manner to ensure efficient clinical flow.

    Possess a preference for performing services that will benefit and help people.

    Possess the social and verbal ability to understand directions and to be communicative with the physician, patient and co-workers.

    Possess the ability to deal tactfully and effectively with patients (and their families), co-workers, hospital staff and the physician.


    Physical Requirements


    Perceive the nature of sounds by the ear, express or exchange ideas by means of spoken word, perceive characteristics of objects through the eyes, extend arms and hands in any direction, seize, hold, grasp, turn, or otherwise work with hands, pick, pinch, or otherwise work with fingers, perceive such attributes of objects or materials as size, shape, temperature, or texture; and stoop, kneel crouch, and crawl.

    Must be able to lift 25 pounds maximum with frequent lifting, carrying pushing, and pulling of objects weighing up to 10 pounds.

    Continuous sitting. Must be able to identify, match, and distinguish colors.
