Nurse Practitioner Position at InnovAge

About the Company: InnovAge is dedicated to providing customized healthcare and social support to seniors through its Program of All-inclusive Care for the Elderly (PACE), offering an alternative to nursing facilities.

Job Description:

• Provide primary care to enrolled participants by evaluating physical complaints and providing treatment
• Perform blood draws, EKGs, dressing changes, and other nursing interventions
• Formulate assessments and treatment plans, including prescription management
• Educate staff, participants, and caregivers on healthcare issues and treatments
• Manage medical needs of participants in various care settings
• Offer emotional and spiritual support to participants
• Handle medical emergencies as needed
• Ensure accurate documentation of medical care in compliance with policies
• Identify and correct problems in medical records
• Other duties as assigned

Requirements:

• 5+ years' experience as a Registered Nurse in elderly care
• Bi-lingual
