    Job Overview

    We are seeking a skilled Home Hospital Charge Nurse to join our team at Mass General Brigham. This is a full-time, ongoing position with a 36-hour workweek, working 12 hours a day.

    About the Role

    This role will service NWH, BWH, and BWFH and will work to provide care in patients' homes. The ideal candidate will have a strong background in nursing, excellent communication skills, and the ability to work independently.

    Responsibilities

    • Plan and implement patient care plans that meet individual needs and ensure optimal patient outcomes.
    • Conduct thorough assessments of patients' conditions and develop comprehensive care plans.
    • Teach patients and families about self-care techniques and provide education on disease management.
    • Maintain accurate records and documentation of patient interactions and care provided.
    • Collaborate with interdisciplinary teams to coordinate patient care and referrals.

    Requirements

    To be successful in this role, you will need:

    • A current Massachusetts license as a Registered Nurse.
    • At least one year of recent experience in acute or emergency settings.
    • Excellent communication and interpersonal skills.
    • The ability to work effectively in a fast-paced environment.
