    Essential Functions


    • Attends specific community meetings to build relationships with those who attend.
    • Working with program participants, make referrals to community resources to overcome social determinants of health that are adversely affecting them, and follow up.
    • Document these interactions in the program database, if appropriate.
    • Work with the Community Health Worker Training Program grant team.
    • Work with external agencies to evaluate those organizations as potential/ongoing experiential learning opportunities in conjunction with the Program and Data Collection Coordinator
    • Assist with gathering accurate report information by stated deadlines, as needed.
    • Attend community events, representing the grant program and Kettering Health.
    • Create and sustain good relationships with community organizations that can provide resources to program participants.
    • Create and maintain database of resources that are useful for students. Can be done in collaboration with the KH health navigators.
    • Maintain adequate records in project software and assist with agency reports, as needed.
    • Complete all required grant documentation, including Time and Effort, in real time.
    • Follow appropriate KH policies and procedures for grant operations.

Are program participants served by the employee who complete a satisfaction survey rating the interactions as above average or excellent? Is project/participant information documented in a way that is easily usable for required reporting?

    Preferred Qualifications

    Job Requirements

    Education

    High school diploma or GED required.

    Certification as Community Health Worker required.

    Work Experience

    No experience required.


    Preferred Experience:
    * work as a community health worker or similar role
    * working collaboratively within community-based organizations.
    * familiarity with Dayton/Trotwood community
    * public health or heath education experience
    * experience working as part of federal grant

    Licenses, Certifications and Registrations

    Certified Community Health Worker (C-CHW) by the Ohio Board of Nursing or obtain certification within 12 months of employment.
