Responsibilities

About the Position:

During a comprehensive visit lasting up to one hour, Matrix providers assess a member's current health status, medical history, medication adherence, social surroundings, and other potential risks. This deep dive offers valuable insights into the member's overall well-being and health that may be overlooked during routine office visits. The Matrix Comprehensive Health Assessment enhances care quality and allows for the identification and closure of multiple care gaps in a single visit.

Nurse Practitioner/Physician Assistant - PRN – Home Risk Assessments

Type: PRN-Flexible

Location: Home, SNF, Telehealth, and various community spots in Hopewell, VA

Hours: Days, Evening, Nights, weekend, Flexible Scheduling

PAY: $95.00 or $110.00 based on county per Assessment, 25 hours or more/month-Flexible Scheduling

Benefits: PRN providers receive limited Voluntary Benefits, with Mileage Reimbursement provided for all miles after 20 completed assessments/month, plus a No-show payment for 10 booked assessments/month

Matrix Provider – Expectations:

• Conduct Adult/Geriatric Assessments, medical history reviews, diagnosis and treatment, health education, physician referrals, and follow-up, adhering to Matrix guidelines
• Collaborate with a variety of healthcare professionals and individuals involved in the patient's care
• Work in different settings like Home Visits, Skilled Facility Visits, and Telehealth Visits

THIS IS NOT A FULL TIME ROLE – It is PRN/COVERAGE with variable hours

Qualifications

Must Haves:

• Master's Degree or equivalent experience and NP licensure completion
• Current RN and NP or PA licensure in the state of practice with prescription authority or ability to obtain it
• Board certification in a relevant specialty approved by Matrix
• Minimum 1 year experience as a Nurse Practitioner/Physician Assistant
• Strong computer skills and familiarity with EMR software
• Excellent verbal and written communication skills
• Positive attitude, team player, adaptable to change, and outgoing personality

Travel Requirements:

• Valid driver's license for travel and adequate automobile insurance
• Occasional travel may be necessary
