Job summary
To provide comprehensive professional nursing support in a multidisciplinary cardiac surgery clinic,to coordinate care follow up patients with complex disorders across continuumProvide expert instruction consultation healthcare team identify gaps needs patient population implement interventions improve outcomes develop partnerships standardize patient care processes facilitate maintain quality improvement activities.
Responsibilities
*Provide expert nursing VAD patient population through individual assessment evaluation monitoring treatment multiple units Coordinate assess nursing care given VAD patient population multiple hospital clinical units Design monitor evaluate nursing programs individualized patient plans Provide liaison consultation area clinical expertise members healthcare team multiple units services disciplines Participate daily care rounds multidisciplinary staff Document care progress detailed records Initiate participate design maintenance clinical area quality assurance programs activities Support standardization development departmental protocols policies procedures Ventricular Assist Device VAD delivery Patient education community public services including police fire hospitals specialists Plan organize travel arrangements medical contracts necessary long distance travel In coordination Equipment Specialist coordinates distribution management device equipment ongoing supply maintenance Participate ongoing research studies compliance federal organizational regulatory guidelines Supervision Received Received supervision professional practice received designated physicians advanced practice team Functional supervision exercised support staff Expectations Position Demonstrated excellent critical thinking problem solving skills Demonstrated ability work collaboratively positively contribute team approach patient care willingness flex cover colleaguesJob description
