- Complete initial and ongoing assessments of all individuals of 18-person caseload
- Assess patients' conditions as needed
- Send chart observations, and perform evaluation tasks including vital signs and medication reviews
- Assist PA with monthly clinics on site
- Oversee medication administration by unlicensed personnel
- Assist with quarterly Psychiatric clinic
- Ensure appropriate medical services are provided for all IDD individuals
- Assist with medical appointments
- Provide effective communication to patients/clients, their family members, team members, and other health care professionals
- Complete Workers Compensation paperwork and follow up as needed
- North Caolina state license as a LPN
- Proof of current CPR certification
- Compassionate, friendly demeanor
- Excellent communication skills
- Current driver's license and reliable transportation
- Ability to work a flexible schedule
- On call availability
