- Provide Community Based Case Management in North Hudson
- Conduct Community outreach, provider education and client recruitment of newly diagnosed and out of care clients in North Hudson
- Refer clients to medical care and medical case management
- Conduct assessments to determine clients' supportive service needs
- Attend medical and other social service appointments with clients.
- Collaborate with primary care providers and MCM staff to identify newly diagnosed patients in need of Community based case management
- Documentation of baseline viral load, CD4 count, AIDS diagnosis and antiretroviral prescriptions for 100% of clients.
- Attend Hudson County TGA Planning Council
- Participate on Hudson County TGA EHE Task Force
- Maintain working relationships with other community providers
- Observe established administrative procedures
- Assist in preparation of written and electronic reports, maintaining records and statistics of program data as required.
- Assist with group activities, outreach events and other program informal and formal social events.
