Job summary
We invite you to partner with us in our mission to improve mental healthcare.This position is responsible for collaborating with the treatment team to facilitate the appropriate discharge and transfer of clients to the appropriate services and level of care. The Case Manager participates in interdisciplinary meetings, collaboration and communication with outside partners and resources, and with preparing clients for program transfer and discharge.
