- Support clinics by planning and managing patient care in collaboration with providers, staff, and volunteers
- Promptly and accurately update patient charts
- Follow-up on patient lab/diagnostic testing and referrals
- Provide comprehensive and intense case management/support services to our most complex patients
- Complete initial needs assessments, develop individualized service plans, and coordinate and educate patients on treatment adherence
- Help us to make sure that everyone in Waukesha County has the healthcare services they need to thrive
- Valid nursing license - LPN or RN accepted
- Ability to perform duties in accordance with the Nursing Code of Ethics and the Nurse Practice Act of Wisconsin
- Experience and understanding of community-based services, resources, and benefit programs
- Experience working with minority and medically underserved populations
- Ability to speak Spanish is a valuable asset (~50% of our patients are primarily Spanish speaking)
