At Riverside Home Health & Hospice, we have been serving patients and families in Southern Oregon for over 20 years.
In recent years, we have expanded our Hospice services — providing comfort, dignity, and compassionate support during life's most sacred moments.
Every hospice journey begins with one call.We are seeking a Hospice Intake Coordinator who understands that how we respond in that first moment truly matters.
This role plays a critical part in ensuring patients and families feel supported, informed, and cared for from the very beginning.

What You'll Be Doing (High Level)
- Receiving and reviewing hospice referrals
- Speaking with patients and families to gather information and explain services
- Coordinating with physicians and referral partners
- Confirming eligibility documentation and required orders
- Scheduling admissions and coordinating clinician assignments
- Ordering durable medical equipment (DME), medications, and supplies
- Supporting smooth transitions from hospital or facility to home
- Identifying and resolving barriers prior to admission
- Maintaining accurate and timely documentation

- Healthcare, hospice, admissions, or intake experience preferred
