Job Summary
We are seeking a dedicated and compassionate Visiting Registered Nurse (RN) or Licensed Vocational Nurse (LVN) to provide high-quality home health care services. The ideal candidate will possess a strong clinical background, excellent patient assessment skills, and a commitment to delivering personalized care in patients' homes. This role offers an opportunity to make a meaningful difference in the lives of individuals requiring skilled nursing support, including those with complex medical needs, chronic conditions, or recovering from illness or surgery. The successful candidate will demonstrate proficiency with electronic health record systems and adhere to all safety and compliance standards.
Responsibilities
- Conduct comprehensive patient assessments, including vital signs, physical examinations, and evaluation of medical conditions.
- Administer medications, injections, IV infusions, and perform wound care such as suturing and ostomy management in accordance with physician orders.
- Provide specialized care for patients with complex needs such as dialysis, ventilator management, or infusion therapy.
- Educate patients and their families on disease management, medication administration, and health coaching strategies to promote independence.
- Document all patient interactions accurately within electronic health record (EHR) systems such as Epic or Cerner while maintaining HIPAA compliance.
- Collaborate with interdisciplinary teams including physicians, therapists, and case managers to develop and implement personalized care plans.
- Ensure aseptic techniques are followed during procedures; maintain infection control standards at all times.
- Assist with triage calls and urgent care assessments to determine appropriate levels of intervention or emergency response when necessary.
Qualifications
- Valid RN or LVN license in the state of practice with current CPR certification.
- Prior experience in home health nursing is highly preferred; experience in hospital medicine, ICU, emergency room, or critical care settings is advantageous.
- Proficiency with EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks.
- Knowledge of medical terminology, ICD coding (ICD-9/10), CPT coding, and documentation standards.
- Experience working with individuals with developmental disabilities, memory care needs, or behavioral health challenges is desirable.
- Strong clinical skills including IV insertion, medication administration, catheterization, tube feeding management, and aseptic technique.
- Ability to work independently while demonstrating excellent communication skills and cultural sensitivity.
- Familiarity with infectious disease care protocols, hospice & palliative medicine principles, and post-acute care practices is a plus. Join our team to deliver compassionate home health services that enhance patient well-being through expert nursing care delivered in the comfort of their homes
