About the Job
The Practice Transformation Manager at CINQCARE is responsible for leading and supporting the implementation of strategies to improve patient outcomes and office efficiencies across various practice settings. This role involves managing a team, analyzing data, and facilitating clinical processes to ensure the success of population health initiatives. The manager works closely with care teams to identify gaps, provide feedback and education, and guide the integration of technology tools into workflows. They also collaborate with other functional areas within CINQCARE and external resources to drive performance improvement and ensure compliance with regulatory guidelines. The ideal candidate possesses a bachelor's or master's degree in healthcare or a related field, clinical licensure, and experience in quality improvement and primary care settings. Leadership, strategic thinking, collaboration, and a commitment to creating a positive work environment are essential qualities for this role.
