    As a Team Leader, you will work within our Program of Assertive Community Treatment (PACT) team located in Cherry Hill, Camden County NJ.

    Schedule: Monday-Friday 8:30am-5:00pm; 30 min unpaid lunch

    **Responsibilities:**

    Functions as a practicing clinician on the team

    Maintains a high level of team moral and conveys the philosophy of the principles of PACT

    Organizes and supervises the day-to-day clinical operations of the team

    Schedules staff work hours and on-call hours to assure appropriate coverage and immediate attention to clients changing needs

    Coordinates the client enrollment process in conjunction with the Psychiatrist and Program Director and provides prompt feedback to the referral source

    Carries out quality-assurance activities and medical records management

    Communicates, evaluates and reports to the Program Director the teams ability to provide services

    Maintains constant communication with the PACT director/coach around team functioning and service delivery

    Empowers the team by modeling strong leadership and conveying the philosophy and principles of PACT;

    Creates a climate that supports the dynamic interaction and participatory process of the team and encourages the establishment of team identity;

    Ensures an equal distribution of team responsibilities;

    Keeps the team focused to complete daily organizational meetings efficiently and effectively;

    Coordinates data collection and reviews the completion of all documentation, including clinical assessments

    Promotes cross-training/education among various disciplines on the team;

    Facilitates a productive decision-making process around client needs and the treatment planning process

    Facilitates daily team meetings to review PACT caseload and clinical plan

    Co-Facilitates staff meetings along with the program Director

    Assists Director and is independently responsible for data collection and reporting for monthly, quarterly. And annual reporting.

    **Qualifications:**

    Masters degree in Social Work or related field;

    Clinical licensure (LCSW, LPC, LCADC, etc.);

    Three years supervisory experience in Mental Health and/or Addictions field. OR

    RN with 3 years of supervisory experience in a Mental Health and/or Addictions nursing.

    Valid NJ Drivers License.

All candidates, if hired, will be expected to cooperate with Oaks Integrated Care and The Department of Human Services in all inspections and investigations.
