    NorthCare is seeking an experienced Nurse Care Manager to join our team. As a key member of our integrated care services, you will play a pivotal role in developing and implementing personalized care plans for individuals facing serious mental illness and addiction disorders.

    **Responsibilities:**

    • Collaborate with consumers to assess their conditions and needs, developing and supervising personalized care plans.
    • Conduct comprehensive nursing assessments to gather physical and behavioral health information.
    • Advocate for consumers and their families, linking them to resources and empowering them to improve their overall health and wellness.
    • Utilize motivational interviewing skills to empower consumers towards better health outcomes.
    • Lead interdisciplinary treatment team meetings to review assessments, update care plans, and determine follow-up frequency.
    • Maintain positive working relationships with primary care providers and social service agencies, collaborating to obtain resources and meet consumer needs.
    • Comply with agency policies and procedures, ensuring documentation deadlines and productivity expectations are met.

    **Requirements:**

    • At least 2 years of experience as a registered nurse (RN) in a healthcare setting.
    • Experience working with consumers with serious mental illness and addiction disorders.
    • Ability to work collaboratively as part of an interdisciplinary team.
    • Strong communication and interpersonal skills.

    **Salary:** $80,000 - $110,000 per year, depending on experience.
