PURPOSE:
The Licensed Practical Nurse (LPN) for the day program provides personal care assistance, medical attention, and administration of physician prescribed medication or PRN if approval is in place.
The LPN is responsible for adherence to the state board of nursing standards and Division of Mental Health and Developmental Disabilities, Illinois Administrative Code Title 59, Rule Medications and Medical Care (in a Developmental Training Program).
Reference must also be made to Rule 116 Administration of Medication in Community Settings.ESSENTIAL DUTIES AND RESPONSIBILITIES:
The list of essential duties and responsibilities, as outlined herein, is intended to be representative of the task to be performed.
The omission of an essential function does not preclude management from assigning duties not listed herein is such duties are a logical assignment to the position.1.
Administer medications and treatments as specifically prescribed by a physician for each individual requiring this support.2. Ensure all medications are secured from unauthorized access.3. Observe for signs of illness, dysfunction, or maladaptive behavior that warrant medical or psychosocial intervention.4.Review Medication Administration Records (MAR) for accuracy, ensuring medication blister packs, bottles or containers match the standing physician order, correcting all errors prior to administration.5.
Maintain first aid supplies and additional inventory needs (i.e., standard PRN medications, gloves, cups), anticipating needs when placing order and replacing expired or worn items.6.
Monitoring special diets and proper feeding techniques.7.Advise the director and program staff of any medical problems that may hinder a participant's progress or that affect participation in program.8.
Ensure safety practices are observed with lifting, transporting, and transferring when applicable.9. Adhere to universal/standard precautions in safety and cleaning measures.10. Complete written reports and other documentation (i.e., progress notes, med error report, incident report).11. Participate in committee meetings as desired, requested or assigned (i.e., Human Rights).12. Make appropriate, accurate judgements including handling of emergency situations.13. Communicate effectively, both verbally and in writing with persons served, families/guardians, other service providers and staff.14. Use a team approach to problem solving as needed (i.e., case management, program, and residential staff).15. Maintain professional and technical knowledge with ongoing training.16.Successfully complete Little City required training, re-training and any additional training as required by policy, practice, or assignment by immediate supervisor.
This included American Heart Association certification in First Aid and CPR and approved CPI training.17. Perform other duties as assigned.Qualifications
MINIMUM QUALIFICATIONS:
High School Diploma or GED and graduation from an approved practical nursing program, required. Must possess a current, valid licensed practical nurse license in the state of Illinois. Two years of licensed practical nursing experience preferred and experience in developmental disabilities field strongly desired.
PHYSICAL REQUIREMENTS &
WORK ENVIRONMENT:
While performing the duties of this job, the employee is subject to standing for prolonged periods; frequently walks, stands, grasps, lifts, holds, or feels objects; occasionally stoops, kneels, crouches, or crawls.
The employee frequently is required to use manual and finger dexterity and eye-hand coordination when working with persons served and handling office or medical equipment.
The employee is subject to assist and support up to 50 pounds with varying amounts of assistance on a reoccurring basis.
The employee is required to have corrected vision and hearing within normal range and the ability to operate equipment. Must have the ability to perform CPR and CPI. Occupational exposure to blood, body fluids, communicable diseases or other potentially infectious substances when performing personal care (Category III). Occasional exposure to cleaning agents and disinfectants. Day program environment with a noise level of normal to loud.The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
