    Summary



    The VA Northeast Ohio Healthcare System is recruiting for a Registered Nurse (Care Coordinator). The Registered Nurse will function as a RN Care Coordinator in Hematology/Oncology Patient Care within Medicine Service.



    Duties


    Summary of Duties:

    • Serves as point of contact for patients with cancers who are under the direct care of a hematologist/oncologist.
    • Coordinates and expedites care from diagnosis to treatment and its complications, and ultimately on to survivorship/end of life care issues.
    • Analyzes and appropriately manages laboratory and radiology results and provides information to physicians and patients as appropriate.
    • Utilizes databases to provide population management and to track patient care through all episodes of care.
    • Confers with other services to develop policies and protocols that promote standardization of multidisciplinary care for diagnosis specific cohorts of patients.
    • Collaborates with Quality Management and Patient Safety leadership to assure best practices are adhered to.
    • Collaborates with physician and nursing staff to develop research projects applicable to the hematology/oncology patient population.
    • Advocates for holistic care through patient/family education and physician interaction and implements an educational plan to meet the patient care needs of the Hematology/Oncology patient. Patient/family education includes pre-therapy, including pre-infusion, teaching.
    • Demonstrates sound judgment, flexibility, and strong decision-making skills in delivery of patient care and readily identifies patient complications.
    • Readily identifies ethical dilemmas and appropriately consults provider, multidisciplinary team, nurse manager or seeks ethics consultation.
    • Collaborates with interdisciplinary team for admissions, transfers, and discharges of patients throughout their outpatient experience.
    • Completes comprehensive nursing documentation according to standards.
    • Treats all patients and families with dignity and respect.
    • Demonstrates responsibility and accountability for own nursing judgments and actions.
    • Ability to function independently as well as with team members.
    • Participates in unit-based activities such as staff in-services and meetings to promote communication, education and knowledge among staff.
    Preferred Experience:
    • Minimum 3-5 years RN experience in outpatient clinical setting
    • Case Management experience; evidence of coordinating complex patient care
    • Oncology Experience
    • Strong critical thinking skills
