    We are seeking an experienced:

    HOME CARE PHYSICAL THERAPY ASSISTANT (PTA)

    Part Time

    Job Description:

    • Provides patient care services and direct treatment as assigned by the Physical Therapist.
    • Updates care plans in coordination with the therapist and physician, focusing on patient progress.
    • Plans monthly patient re-evaluations with the therapist, coordinating more frequently if needed.
    • Documents patient care data accurately and thoroughly, reflecting improvement or decline in patient condition.
    • Submits daily/weekly patient visit schedules to the Scheduler.
    • Communicates with internal and external contacts as necessary.
    • Discusses treatment progress with the therapist, patient/family, and physician when needed.
    • Coordinates referrals to appropriate disciplines and supervisory visits from the therapist.
    • Attends patient care conferences, meetings, and mandatory in-services, reviewing minutes of any missed conferences.

    Requirements:

    • Completion of an accredited physical therapy assistant program and current State licensure.
    • Minimum one year's therapeutic experience; preference for two years.
    • Strong knowledge of physical therapy theory and concepts, with ability to treat patients under a physician's order.
    • CPR certification and excellent communication and organization skills.
    • Twelve hours of continuing education annually.
    • Ability to accept travel and working conditions as assigned.
