- Conduct Adult/Geriatric Assessments, medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols
- Collaborate with healthcare professionals and caregivers for comprehensive patient care
- Work in various settings including Home Visits, Skilled Facility Visits, and Telehealth Visits
- Master's Degree required OR relevant experience and NP licensure
- Current RN and NP licensure with prescription authority
- Board certified in a Matrix approved specialty
- BLS, ACLS, or CPR certification
- Minimum 1 year experience as a Nurse Practitioner
- Strong computer skills and familiarity with health record software
- Excellent communication skills
- Flexible and adaptable mindset
- Valid driver's license, car, and adequate insurance
- Ability to travel to patient locations

Matrix Provider – What to Expect:
Nurse Practitioner - PRN/Part Time - Home Risk Assessments
Type: PRN/Part Time - Flexible-Nurse Practitioner
PAY: $90.00 or $95.00 per completed Assessment (based on county)
Location: Home, SNF, Telehealth, and community environments in Springfield, Ohio
Hours: Day, Evening, Night shifts available - Flexible Scheduling
Benefits: 401(k) and limited Voluntary Benefits for PRN/Part Time providers
Mileage Reimbursement plan
