    POSITION:

    • Physical Therapist (Home Health Agency)
    • This is a full-time position, which requires a minimum of 30 visits (and/or equivalent work activities) per week.
    • Part-time and PRN applicants are welcome to apply.

    LOCATION:

    Emporia, Kan.

    COMPENSATION:

    $53.00 to $59.00 per visit, depending on experience.

    POSITION SUMMARY:

    Love My Nurse seeks a skilled and compassionate Physical Therapist to join our team. As a Physical Therapist with Love My Nurse, you work closely with patients in their residences providing individualized rehabilitative care. Your role will include conducting assessments, developing treatment plans, and delivering therapy services to help patients regain mobility, reduce pain, and improve overall quality of life. The ideal candidate will have strong clinical skills, excellent communication and client care abilities, and a passion for delivering outstanding patient care.

    KEY RESPONSIBILITIES:

    • Conduct comprehensive evaluations of patients' physical conditions, including medical history, mobility, strength, and range of motion assessments.
    • Develop personalized treatment plans based on evaluation findings, in consultation with the interdisciplinary team, to address patients' specific needs and goals.
    • Provide therapeutic interventions and exercises to improve mobility, promote pain management, enhance physical strength, and restore functional abilities.
    • Instruct patients and their families on appropriate exercises, functional training techniques, and safety measures to enhance the progress made during in-home therapy sessions.
    • Monitor patients' progress and regularly reassess treatment plans to maximize functional outcomes and adjust as needed.
    • Collaborate with the interdisciplinary team, including physicians, nurses, occupational therapists, and speech therapists, to ensure coordinated and holistic care is provided.
    • Maintain accurate and timely documentation of all therapy services provided, including evaluation reports, progress notes, and discharge summaries, according to agency guidelines and regulatory requirements.
    • Communicate effectively with patients, families, caregivers, and other healthcare professionals to establish rapport, address concerns, and ensure a collaborative approach to the care plan.
    • Adhere to ethical, legal, and professional standards, including maintaining patient confidentiality, practicing within the scope of practice, and participating in continued professional development activities.
    • Keep up to date with the latest research, techniques, and evidence-based practices in physical therapy, integrating new knowledge into clinical practice to improve patient outcomes.

    QUALIFICATIONS:

    • Must hold a valid license to practice as a Physical Therapist in the state of Kansas.
    • Doctorate degree in Physical Therapy from an accredited program.
    • Prior experience working as a Physical Therapist, preferably in a home health or related setting.
    • Strong knowledge and understanding of physical therapy principles, techniques, and methodologies.
    • Excellent clinical assessment, diagnostic, and treatment planning skills.
    • Ability to effectively communicate and collaborate with patients, families, caregivers, and interdisciplinary team members.
    • Proficient in using electronic health record systems and other relevant software for documentation and communication purposes.
    • Demonstrated ability to work independently, manage time effectively, and prioritize tasks to meet patients' needs and agency requirements.
    • Strong commitment to maintaining ethical standards, patient confidentiality, and professional development.
    • Valid driver's license with access to personal transportation and ability to travel to patients' residences.
    • Provide binders showing current automobile insurance meeting state's minimum requirements.

    SUPERVISORY RESPONSIBILITY:

    This position supervises Physical Therapist Assistants.


    WORK ENVIRONMENT:

    This job operates primarily in patient homes where there are numerous uncontrolled variables.

    EXPECTED CORE HOURS/DAYS OF WORK:

    Expected core hours of work are 8 a.m. to 5 p.m., Monday through Friday. Evening and weekend work may be required as job duties demand.


    TRAVEL:

    • Up to two to three (2-3) days of the week can potentially involve day-travel to assist others in providing care (travel allotments and mileage included).
    • Occasional overnight travel may be expected, but is highly infrequent.

      • Employee share of premium(s) deducted from paycheck on a pre-tax basis, lowering tax liability.
    • Vacation | Two (2) weeks per year. One (1) week available after 90 days, and another awarded at six (6) months. Thereafter, two weeks are awarded together on the anniversary date of the employee's hire.

    MILEAGE/TRAVEL ALOTTMENT:

    • Love My Nurse pays $0.585 per mile for travel inside a service area. "Commute" (defined as the travel to the first appointment and from the last appointment) is not included in paid mileage.
    • Love My Nurse pays a minimum of a $50.00 travel allotment when providing care in another service area.
