    ESSENTIAL DUTIES AND RESPONSIBILITIES:
    • Provide intensive case management services utilizing the Transition to Independence Process (TIP) model. Job functions include, but are not limited to intake, assessment, discharge planning, and follow-up. Provide services that guide and advocate for youth in planning future goals and setting attainable objectives. Teach youth the required skills to assist with decision-making and establishing a readiness to address barriers. Provide support, encouragement, and accountability to youth in their daily activities to fulfill their individualized treatment plan. Prepare youth and families for discharge so they can maintain and build upon their plan achievements. Ensure that case management services are in a culturally competent manner according to CHF's Mission, Principles, and Core Values.
    • Responsible for the overall supervision and safety of youth. Supervise youth, maintain order, remain alert, and exercise appropriate judgment at all times (i.e. at meal times, on the residential floors, in common areas, during recreation activities, and escorts). Assist youth in emergencies as directed by supervisor. Defuse physical and verbal confrontation with youth served, using appropriate verbal behavioral de-escalation techniques and/up to advanced physical interventions. Must be certified and up to date in Nonviolent Crisis Intervention Training.
    • Assist youth and their families with the necessary community resources/referrals to advance youths plans, address legal obligations, and meet program requirements. Escort youth to court, school, and other appointments. Link youth to CHF on-site services including therapeutic and health services, GED classes, residential substance abuse treatment, Job Readiness training, and Life Skills instruction.
    • Communicate pertinent information with staff and youth throughout each shift. Communicate respect, mutual support, honesty, and care to all youth and staff. Take an active part in shift transition and provide vital information about youth and referrals.
    • Complete all documentation neatly and accurately (i.e. case records, incident reports, data entry, billing abuse reports). Ensure that documentation is completed promptly and meets the standards established by state license requirements, funding sources, and agency policies, including filing and tracking data for grant objectives. Utilize and monitor youth information in all applicable human service database systems including but not limited to ETO, SAMIS, HMIS, and JJIS.
    • Maintain a clean and safe environment. Assist and supervise youth in general cleanliness of all residential living and common areas. Ensure that work areas are clean and organized before, during, and at the end of the shift. Report all maintenance concerns to the Operations Department.
    • Drive company vehicles to escort youth to and from activities, hospital visits, school, and various other appointments, etc.
    JOB REQUIREMENTS:

    EDUCATION:

    Bachelors Degree in human services field.

    EXPERIENCE:

    Minimum of one (1) year of experience in social services or related experience to provide Case Management, linkages to community service, and to provide services to program participants.

    SCHEDULE:

    Tuesday 8:00 am - 6:00 pm; Wednesday 12:00 pm - 10:00 pm; Friday & Saturday 8:00 am - 6:00 pm

    Sunday & Monday 9:00 am - 7:00 pm; Tuesday 12:00 pm - 10:00 pm; Wednesday 10:00 am - 8:00 pm.

    SKILLS:
    • Must be a self-starter and team player with the ability to work well with others.
    • Must have demonstrated ability to maintain order and exercise appropriate judgment in crises including the physical ability to intervene in critical or emergencies.
    • Knowledge of adolescent development and ability to build positive rapport with young people.
    • Excellent communication skills as demonstrated by concise, timely, and professional correspondence, both written and verbal.
    • Demonstrated understanding of and sensitivity to culturally diverse populations, conflict resolution skills, active listening skills, and computer literacy are required.
    TRANSPORTATION:

    Must have a valid FL drivers license and a good driving record. May need to transport clients/staff if required using agency transportation.

While performing the duties of this job, the employee may work occasionally in outside weather conditions. The noise level in the work environment is usually moderate to loud. Must be able to lift and carry up to 20 lbs. Must frequently move about both indoors and outdoors and on occasion, bend, sit, and stand for prolonged periods. Some weekend/holiday work may be required.

    SUPERVISORY DUTIES:

    This position does not perform supervisory duties.

    EQUIPMENT USED:

    Must be able to utilize basic word processing, email, and internet programs. Must be able to communicate effectively both orally and in written form.
