    Job Description Job Description Fraser is currently seeking a highly motivated and experienced individual to join our team as a Program Coordinator/Designated House Coordinator.

    Applicants with strong communication skills and experience coordinating services are strongly desired.***You must be 245D qualified for this position. Find below for the listed requirements.


    Qualifications:


    Candidates must qualify based on one of the following three as assigned by 245D, which includes a combination of work experience and education.

    HS Diploma or EquivalentBA/BS in Human Services or related field with 1 year of full-time work experience (or equivalent) providing direct care services to persons with disabilities or persons age 65 years and older.

    Associate degree in Human Services or related field with 2 years of full-time work experience (or equivalent) providing direct care services to persons with disabilities or persons age 65 years and olderA minimum of 50 hours of education and/or training related to human services and disabilities with four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 or older.


    Note:
    a qualifying Designated Coordinator under 245 D must provide Supervision.

    Responsibilities:


    Assure necessary staff coverage, and recommend staffing, to meet the needs of the clients servedCoordinate and/or complete all household repairs and other upkeep.

    Support other Program Coordinators with on call duties as directed.
    Coordinate the activities of daily living including personal care, hygiene, and grooming.
    Provide direct cares to our clients and supervisory duties.
    Collaborate with medical professionals to coordinate health and medical needs for each client served.
    This position is on-call and will serve as a part of an on-call rotation.

    • Starting pay $21 an hour and could go up depending on education and experience with a $750 hiring bonusProgram Coordinator
