    Job Title:

    A Physical Therapist at VNS Health

    About the Role:

    As a Physical Therapist with VNS Health, you will provide personalized home-based rehabilitation therapy to patients recovering from surgeries, illnesses, or accidents. You will work within an interdisciplinary care team to assess patient needs, develop treatment plans, and adjust care according to evolving patient conditions.

    Main Responsibilities:

    • Practice independently in the community as part of an interdisciplinary care team.
    • Conduct comprehensive assessments of patients by determining their needs, developing treatment plans, and evaluating outcomes.
    • Deliver personalized rehabilitation therapy to patients in their homes or care facilities.
    • Evaluate evolving patient needs and respond with plan of care adjustments.

    Qualifications:

    • Minimum of one-year physical therapy experience or successful completion of probationary period as a VNS Health Rehab Intern
    • Licensure to practice as a Speech Language Pathologist/Audiologist in New York State
    • Valid driver's license required for all areas outside the borough of Manhattan
