    About the Job

    About Us: A Hug Away Healthcare is a leading provider of home health and hospice services. We are seeking a compassionate and experienced Home Health and Hospice Nurse to join our team. This role involves providing high-quality care to patients in their homes and managing their cases effectively using the Kinnser EMR system.

    Key Responsibilities

    • Provide comprehensive nursing care to home health and hospice patients, including assessments, care planning, and implementation of care plans.
    • Utilize the Kinnser EMR system to document patient care accurately and efficiently.
    • Perform case management duties, including coordinating care with other healthcare providers, managing patient records, and ensuring continuity of care.
    • Educate patients and their families about their conditions, treatment plans, and available resources.
    • Monitor and evaluate patient progress, adjusting care plans as necessary.
    • Administer medications and treatments as prescribed by physicians.
    • Conduct regular home visits to assess patient needs and provide necessary care.
    • Communicate effectively with patients, families, and the healthcare team to ensure optimal patient outcomes.
    • Participate in interdisciplinary team meetings to discuss patient care and develop comprehensive care plans.
    • Ensure compliance with all regulatory requirements and company policies.

    Qualifications

    • Valid Registered Nurse (RN) license in the state of Texas.
    • At least 1 year of experience in home health and hospice nursing.
    • Proficiency in using the Kinnser EMR system.
    • Strong case management skills and experience.
    • Excellent organizational and time-management skills.
    • Compassionate and patient-centered approach to care.
    • Strong communication and interpersonal skills.
    • Ability to work independently and as part of a team.
    • Valid driver's license and reliable transportation.

    Preferred Qualifications

    • Certification in hospice and palliative care.
    • Experience in the Kingwood, TX area.
    • Familiarity with Medicare and Medicaid regulations.
    • Experience working with children is highly preferred but not required.
