- Provide patients with timely and accurate reminders related to preventative care services
- Provide pre-visit planning for high-risk patients to include gaps in care such as immunizations and preventive screening exams
- Complete provider notification of gaps in care for the individual patient
- Provide comprehensive care management to improve patient and practice outcomes
- Partner with co-workers to provide education regarding specialty medicine practices
- Triage patient needs, assign referrals, and assist patients as needed
- Maintain accurate documentation of referrals, specialist visits, and ED/Hospital stays and document interventions
- Review monthly dashboards and data to conduct and participate in data analysis process
- Complete additional duties as assigned within the scope of the care coordinator practice \
- Education: Graduate of an accredited nursing program
- Licensure and Certifications: Arkansas LPN license and CPR, required.
- Experience: Minimum 3 years' clinical experience as an LPN, required.

Position Summary
The role of the Care Coordinator - Licensed Practical Nurse reports to the Director Value Based Programs.
This position is responsible for the coordination of practice transformation initiatives for a group of high-risk patients within primary care locations.
