About the Job

We are seeking a skilled Physical Therapist to join our team as a Home Health Physical Therapist. This is a per diem opportunity that involves performing home health visits for senior living office in Raleigh, North Carolina.

Responsibilities:

Make home visits to clients in designated geographic territories.
Perform diagnostic tests and measurements.
Implement integumentary protective interventions.
Develop and implement individualized care plans.
Educate clients and family members on rehabilitative care.
Accurately document client care management and services provided.

Requirements:

A current North Carolina Physical Therapist license.
A minimum of one year of recent work experience as a Physical Therapist.
Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
Ability to work independently and manage time effectively.
Strong interpersonal, organizational, and problem-solving skills.
Solid computer skills; prior experience with electronic medical records (EMR) preferred.
