Description

This Nurse Practitioner is responsible for assisting patients referred to Hudson Valley Medical Health Choices Palliative Care Consultation Service in managing their medical conditions by helping to coordinate goals of care and support services in collaboration with primary care providers and specialty care physicians. Provide Hospice informational and eligibility assessments when needed.

ESSENTIAL FUNCTIONS

Assist patients with complex illnesses, recovering from recent hospitalization, or managing multiple comorbidities.
Coordinates comprehensive, holistic, individualized plans of care to maximize quality of life.
Builds relationships with patients, families and providers.
Provides education on and assists with ensuring patients have effective pain and symptom management.
Monitors medication compliance and hospital/ER utilization
Helps patients and families understand their clinical conditions and navigate treatment options.
Help patients clarify goals of care and address non-clinical issues that impact quality of life and outcomes.
Initiate discussions and education regarding advanced directives and DNR, providing information and answering patient/family questions.
Perform HVMHC informationals, eligibility assessments and admissions as needed.
Collect, analyze and summarize clinical data and metrics as requested.
Identifies community resources to assist family with meeting their goals of care.
Follows-up with patients /caregivers after RN/Provider visits to ensure positive outcomes and goals are met.

WORK ENVIRONMENT/PHYSICAL DEMANDS

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

Working conditions include working within assigned settings, including but not limited to Hudson Valley Hospice offices, patient/family homes and assisted living communities
Ability to carry up to twenty-five pounds of supplies and/or materials.
Infrequent duties involve potential for direct exposure to blood or body fluids.
Capability to work a flexible schedule to meet the needs of the patients.
Ability to provide transportation for self and access public buildings to provide community education.
Valid driver's license in New York State.

PREFERRED EDUCATION & EXPERIENCE:

Graduate from accredited nursing program with Master of Nurse Practitioner
Valid RN & NP license
DEA License
Minimum 2-3 years nursing experience as a Nurse Practitioner required
Previous Palliative Care / Hospice experience preferred
CHPN certification preferred - willing to obtain after one year of employment

Hours: Monday-Friday (8:30am-4:30pm)

Pay: $133, ,000 per year

FLSA: Exempt
