    Description

    Countryside Care Center is a provider of exceptional short-term rehabilitation and long-term care services. Our skilled nursing team works closely with residents and their families to develop personalized treatment plans that promote hope and strength. We strive to deliver quality care by incorporating progressive therapies and a compassionate approach into our resident's care plans. Our interdisciplinary team includes nursing staff, therapists, a resident dietitian, and social services, all working together to address each individual's specific needs and therapy goals.

    We are seeking a full-time compassionate and skilled Licensed Vocational Nurse (LVN) to join our team at a Skilled Nursing Facility. As an LVN, you will be responsible for providing direct nursing care to our residents under the direction of a Registered Nurse (RN), and ensuring that the facility meets all applicable regulations and standards.

    NOC shift is available at this time.

    Responsibilities:

    - Provide direct nursing care to residents, including administering medications and treatments, monitoring vital signs, and assessing and documenting changes in their condition

    - Assist with the development and implementation of individualized care plans based on residents' needs and goals

    - Coordinate care with other members of the healthcare team, including physicians, therapists, and other healthcare professionals

    - Ensure that all documentation and charting is accurate, complete, and up-to-date

    - Assist with activities of daily living (ADLs) as needed

    - Educate residents and families about their healthcare needs and treatments

    - Respond to emergencies and provide first aid as needed

    - Adhere to all policies, procedures, and regulations governing nursing care in the facility

    Qualifications:

    - Valid and current LVN license in the state of California

    - Graduate of an accredited nursing program

    - Minimum of 1 year experience in a skilled nursing facility, acute care setting, or in-patient rehabilitation or in-patient behavioral health setting

    - Minimum 1 year PCC experience

    - Strong clinical skills, including assessment, planning, and implementation of nursing care

    - Excellent communication and interpersonal skills

    - Ability to work independently and as part of a team

    - Strong critical thinking and problem-solving skills
